SAGE-SR (SELF-REPORT DIAGNOSTIC SCREENING)
SAGE-SR is the world’s first completely self-report comprehensive diagnostic assessment. It consists of 70 short screening questions written at a 5th-grade reading level, with no open-ended questions. Designed to be user-friendly for both client and clinician, individual assessments take 10-20 minutes depending upon the number of screening questions endorsed. SAGE-SR can be administered in a waiting room or at the client’s home. Reports are available online and in many instances can be sent directly to the client’s EHR.
SAGE-CI (CLINICIAN DIAGNOSTIC INTERVIEW)
For deeper evaluation, the SAGE-CI is a highly-structured interview which can be administered by any primary care clinician. The SAGE-CI can stand alone, prompting clinicians to ask questions in a structured interview, or it can build upon the patient’s stored responses from the SAGE-SR. The resulting report provides the clinician a specific diagnosis with relevant ICD-10 codes. These interpretive clinical reports can be sent immediately to the patient’s EHR, read online, or printed out before a clinician visit.
